You know what sucks most of all? It's got two perfect mods, on a 15% against hexed foes, req13 hammer -.- You can salvage it and you'll definitely lose one of those two mods...possibly both.

I also wondered how someone could apply poison on a hammer, but I finally concluded that sometimes, it's best if we don't know the answer >_>
